Home Health Care Agency
in New Jersey.

At the assumed 80 completed RN visits at $200 per visit and 800 aide care hours at $45 per hour, the New Jersey occupation-wage scenario earns $52,000 fees before the separate 2% expected credit loss. It produces -$10,611 mature monthly EBIT and -$10,444 cash after capital maintenance. At the 85-RN-visit/850-aide-hour held-mix paid-resource ceiling, EBIT is -$7,790. The approximate EBIT condition of 98.78 planning equivalents exceeds the paid ceiling; more volume at the same mix is not a deliverable remedy. Fees, care volume, travel, clinical work rates, premises and coverage contracts are assumptions. The director clock assumes eligible combined clinical direction and agency administration only where permitted; its 40 monthly standing hours cover governance/administration, with clinical activity added once. Any combined appointment depends on the regulator’s actual employee-and-contractor count, qualifications, required direct employment, duties and qualified alternate administration. The 8.75 wage-pool FTE excludes availability-provider personnel and does not establish a statutory count. Clerical medical administration is not the qualified administrator. Add a separately qualified paid pool when required. State-specific authority and actual qualified availability remain prerequisites.

What changes at New Jersey wage anchors?

At the assumed 80 RN visits and 800 aide care hours each month, priced at $200/visit and $45/care hour, earned fees stay $52,000.00. Direct May 2025 occupation medians calculate $47,835.23 loaded payroll, -$10,610.70 mature EBIT and -$10,444.03 cash after capital maintenance, before receivable movement, financing, income tax and distributions. Payroll is $2,873.69 more than the identical national-wage roster. U.S. Bureau of Labor Statistics: May 2025 occupation wages. Rent, patient activity, prices, employer additions, opening and availability amounts remain assumptions; the result is a wage sensitivity, not an observed state startup cost.

The three RN lines are separate allocations, with no simultaneous reuse of field, clinical-director or alternate-response hours. The director pool also includes qualified agency administration only where a combined appointment is allowed, within its existing clock; otherwise add a separate eligible paid manager. Clerical administration is a different role. All three occupational medians are direct state observations; none is a national fallback or a city offer. The 18% employer addition is assumed. Check every worker’s eligible duties, jurisdiction, wage rule, overtime, benefits, travel and absence cover against the actual schedule. Wage data and release freshness were checked October 16, 2026. U.S. Bureau of Labor Statistics: May 2025 occupation wages.

Can the paid pools support the financial threshold?

The approximate same-mix EBIT threshold is 98.78 RN-visit equivalents, with ten aide care hours per equivalent at the default mix. At whole completed RN-visit increments, the financial condition is 99 RN visits and 990 aide care hours. The modeled ceiling is 85 RN visits plus 850 aide care hours. The same-mix financial EBIT threshold exceeds the paid-resource plan. Raising volume at this mix cannot meet it within the disclosed capacity.

Capacity includes care, paid intervisit travel, documentation, aide handoffs, fixed work and activity-linked recovery, plus separate clinical direction, alternate response and administration. A monthly aggregate cannot prove compatible individual shifts, immediate qualified response, absence coverage or appropriate care. The 24/7 availability allowance does not create 24-hour bedside services or unlimited active nursing labor. Longer trips, more incidents, staff turnover or additional supervisory needs change both the cost and the achievable care schedule.

What does the historical New Jersey industry row establish?

The 2023 employer-industry row for NAICS 621610 records 988 establishments in New Jersey. The classification includes multiple payer models and services, including hospice and infusion. What does the state wage observation change?

The May 2025 RN occupation median in New Jersey is $51.20 per hour. The aide and medical-administration medians are $17.83 and $23.39. They are cross-industry observations, not offers for this agency’s credentials, shifts or clients.

BLS May 2025 state occupation workbook · retain exact occupation, geography and period when comparing an actual recruiting quote.

The same paid pools cost $47,835 monthly at the New Jersey wage anchors versus $44,962 nationally, a difference of $2,874 including the assumed employer addition. This isolates wage substitution; it does not compare premises, service authority, care demand, achieved fees, travel or availability contracts.

Evidence behind the two-service state scenario
Evidence classWhat this establishes
ObservedMay 2025 RN, broad home-health/personal-care aide and medical-administration wage records in the stated state. Population is background, not care demand.
Derived$52,000 earned fees, -$10,611 EBIT and -$10,444 cash after maintenance follow the disclosed two streams, costs and wage substitution.
AssumedCare volume, fees, visit duration/grouping, travel, records, recovery, paid pools, employer load, qualified on-call contract, an eligible combined qualified director/administrator appointment where permitted, premises, opening/accreditation, loss, collection lag, ramp and buffer remain planning assumptions. The 40 monthly standing director hours include governance/agency administration, and clinical work is added once. The clerical pool is not a qualified administrator; a required separate appointment adds another paid pool.

At 20% fewer completed services—64 RN visits and 640 aide care hours—the same paid roster produces -$19,649 EBIT. The approximate same-mix EBIT condition is 98.78 equivalents versus a paid ceiling of 85. The financial condition does not fit the paid ceiling, so more volume at the same mix is not a feasible remedy.

Three operating sensitivity tests
Held-constant testExact changeMature EBIT / monthCash after capital maintenance / month
20% fewer completed care services64 completed RN visits + 640 aide care hours; fees and every paid pool unchanged-$19,649-$19,482
10% higher wage anchors1.10 × all three occupation wage anchors; paid hours and 18% employer addition unchanged-$15,394-$15,228
20% higher office allowance$2,400 office allowance, 20% above the assumed $2,000; all other inputs unchanged-$11,011-$10,844

Of these declared test sizes, 20% fewer completed care services has the largest absolute EBIT effect: -$9,038 per month relative to this wage reference. The tests have no assigned probabilities and do not establish actual demand, hiring offers or an office quote. Care changes retain whole RN completions and whole aide visit blocks; maintenance cash remains distinct from EBIT and excludes receivable movement, financing, tax and owner distributions.

The national plan holds the shared definitions and financial guide explains the same care-and-cash equations. Named state observations establish only their own authority, provider terms and survey coverage.

Editorial assessment

Fund qualified coverage before relying on billable care

Interpretation of a state wage reference scenario

An agency decision should follow the exact service authority, accepted care and a qualified roster whose full paid work can earn collected contribution. RN visits and aide care hours are different services; field travel, records, protected direction, alternate response and qualified availability cannot be treated as free overhead. A licence or referral pipeline does not establish deliverable paid demand.

The New Jersey wage scenario earns $52,000 fees from 80 assumed RN visits at $200 and 800 aide care hours at $45, before separate credit losses. With field nurses, a full clinical director, qualified alternate response, aides, administration and on-call availability paid, mature EBIT is -$10,611 and cash after maintenance is -$10,444. The approximate same-mix EBIT condition is 98.78 planning equivalents versus a paid ceiling of 85; more volume at the same mix cannot repair the shortfall within that capacity. At 20% fewer completed services, EBIT is -$19,649. State wages are observations. Fees, demand, clinical work rates, travel, all commercial costs and coverage contracts are assumptions; actual license/category, plans, credentials, accreditation and qualified availability remain prerequisites.

The New Jersey wage inputs put the same modeled payroll $2,874 per month above the national reference. The comparison includes the assumed 18% employer-cost allowance. It does not establish a difference in total startup costs or profitability: premises, fit-out, selling prices and demand remain commercial reference assumptions.

Quote the actual authority, accreditation, insurance, qualified appointments and availability arrangement. Reconcile a named client and staff week to completed care, every paid clock and dated collections. Change supported fees, service mix, territory or complete paid costs when the financial condition lies beyond capacity; a larger loss reserve is not an operating remedy.

EBIT includes the modeled paid roster and depreciation, before financing and income taxes. It is not owner take-home pay.

The Census estimate for New Jersey is 9,548,215 people. It grew by 41,861 between July 2024 and July 2025 (+0.44%). This statewide movement cannot identify a viable frontage or the trading pattern of a neighborhood.

Using the same paid roster, New Jersey occupational wages produce $47,835 of monthly loaded payroll. That is +6.39% relative to the identical roster priced with national occupation medians ($44,962). Only wage benchmarks change in this comparison; it does not measure a state’s overall business attractiveness.

Keep the wage difference in proportion. Site, price and demand can still dominate the decision. Choose a city and a catchment before using statewide population to plan daily sales.

Which business is being modeled?

Original ink-and-watercolor illustration of an intermittent RN home visit, with a seated client, a care-plan discussion, closed clean visit bag, blank unidentifiable records and a clear walking route in an ordinary home.

Private-pay agency providing state-authorized intermittent RN visits and supervised home-health aide care, with paid clinical direction and qualified on-call coverage. A private-pay agency earns fees from completed intermittent nursing visits and supervised aide care while funding licensed clinical direction, field travel, records, qualified backup and collection delays.

The qualified director pool includes standing governance and agency administration only where an eligible combined appointment is permitted; the clerical pool is not the qualified administrator. The default delivers 80 one-hour RN visits and 800 aide care hours in 200 four-hour visit blocks. Field RN work is 14 standing hours plus 80 direct-care, 40 travel, 20 documentation and two expected recovery hours: 156 of 173.3333 paid hours. Aide work is 24 standing hours plus 800 care, 100 travel, 30 documentation, 20 handoff and two expected recovery hours: 976 of 1,040 paid hours. Clinical direction uses 70 of 173.3333 hours; the qualified alternate RN response pool uses 16 of 43.3333; medical administration uses 61.4 of 86.6667. These pools are distinct and cannot be counted as simultaneous field care. At the held ratio of one RN visit to ten aide care hours, 85 whole planning equivalents fit all clocks, including 850 aide care hours across 213 visit blocks. Aides use 1,035.88 hours at that ceiling; the next equivalent needs 1,047.4 and fails. Every expected recovery rate grows with proposed activity. These are authored work and scheduling assumptions, not patient demand, legal staffing standards or proof of a deliverable address-level schedule.

The $200 visit and $45 care-hour fees are authored assumptions after concessions and refunds but before the separate 2% credit loss. They do not come from a scope-matched realized-price quote. Required care, actual visit duration, cancellation treatment and clinical authority must be established for each accepted client.

Earned fees are after concessions and refunds but before the separate assumed 2% credit loss. Payment costs are 3% of collectible earned fees. Care supplies cost an assumed $8 per RN visit and $1 per aide care hour. Reimbursed travel assumes 20 miles per RN visit and 12 per aide visit at an authored $0.70 per mile; that rate is not presented as a tax or statutory reimbursement rate. Every paid field, documentation, supervisory and response hour remains in payroll. The $5,500 monthly standing allowance comprises $1,200 insurance, $500 secure scheduling/records, $500 marketing, $500 professional/compliance support, $200 utilities, $100 standing communications and a separate $2,500 qualified 24/7 on-call availability retainer. The retainer purchases assumed availability; actual response work has its own paid RN pool. Neither amount is a provider quote or proof of legally adequate coverage. The $2,000 office allowance and $300 capital-maintenance cash are separate.

Medicare or Medicaid billing, payer certification, therapy, hospice, infusion, medical-equipment sales, continuous bedside or live-in care, pure companionship-only service, staffing contracts, franchises, multiple branches, property purchase and unpaid owner work are excluded. Qualified 24/7 on-call availability is included as an assumed contracted commitment, distinct from continuous bedside care. Actual service authority, required care plans, credentials, director and administrator eligibility, accreditation and availability obligations depend on the jurisdiction and exact services. Selling prices exclude collected sales tax. No price or volume above is presented as a market observation for New Jersey.

Base wages total $40,538 per month. An authored 18% allowance for employer costs adds $7,297, giving $47,835 of loaded payroll. The allowance is a planning shortcut; it is not a state-specific payroll tax calculation or benefits quote for New Jersey.

The director pool assumes an eligible combined clinical-director and agency-administrator appointment only where permitted. Its 40 monthly standing hours cover governance and agency administration; activity-linked clinical work is added once within the same paid clock. Any combined appointment depends on the regulator’s actual employee-and-contractor count, qualifications, required direct employment, duties and qualified alternate administration. The 8.75 wage-pool FTE excludes availability-provider personnel and does not establish a statutory count. The medical-administration pool supplies clerical work, not the qualified agency administrator. If separate qualified roles are required, add a separately paid pool and recalculate. May 2025 BLS medians anchor Registered Nurses (29-1141), Home Health and Personal Care Aides (31-1120) and Medical Secretaries and Administrative Assistants (43-6013). Paid field RN and clinical director coverage each has 40 weekly hours, and the qualified alternate RN response pool ten, totaling 390 monthly RN person-hours at 52/12 weeks per month. Aides have 240 weekly hours, or 1,040 monthly; administration has 20 weekly hours, or 86.6667 monthly. The cross-industry aide occupation includes personal-care work beyond this licensed scope. Medians do not prove hiring costs, credentials or availability. The 18% employer addition is assumed. Split aggregate pools into enough qualified people, lawful individual shifts, leave/relief, travel, training and overtime; a full-time director cannot be silently reused as a simultaneous field nurse. Every operating role is paid, including management or supervision. If the owner performs a modeled role, their compensation occupies that role once; no additional owner draw is included in operating profit.

The middle 50% wage interval describes the occupation’s observed wage distribution. It is not a confidence interval for this business’s total payroll. Allocate the aggregate hours across an actual roster and check wage rules, overtime, leave and employer obligations for the chosen location.

At this roster, a 10% increase in wage rates adds $4,784 per month to loaded payroll. At the reference price and variable margin, it needs about 7.6 additional planning mix equivalents per month to offset it. This sensitivity holds staffing hours and other inputs fixed.

The equipment and premises allowances are the same in all 50 states for this operating format. They are a comparison baseline and must be replaced with local scopes and quotes. Training uses 120 aggregate paid hours at the modeled team’s weighted loaded rate. The refundable deposit is cash tied up, not an operating expense.

$753,307Peak cumulative operating cash deficit · month 60
$110,6702 months of fixed cash costs · assumed buffer
$936,762Opening payments + deficit + buffer

The cash schedule tests 60 months, with sales ramping through 40%, 55%, 70%, 82%, 90%, 96%, 100% of the volume assumption. Full payroll and fixed costs begin in month one. The reserve covers the deepest cumulative operating deficit plus the stated buffer. A buffer is retained cash, not spending and not part of project payback twice.

Earned fees, collectible balances and cash are separate. The default applies a 2% expected credit loss once, then holds 30 average days of collectible earned fees in month-end receivables. Payment costs and other modeled commitments are assumed paid in the earned month; actual invoice, payroll and settlement dates need a dated ledger. Customer advances for undelivered care, refunds and restricted funds remain separate obligations. This wage scenario loses $10,444 mature cash per month; its $936,762 opening-plus-60-month reserve would finance modeled continuing losses. This is not a viability or financing recommendation. Opening payments are not sustainably recovered within 60 planning months. State wages are observations; all commercial, workload and collection inputs remain assumptions. Asset purchases are depreciated over 60 months for this scenario. No sale or deposit recovery is assumed at the end.

The reference mature month loses cash. A 60-month cash reserve is a finite liquidity calculation; it cannot make a permanently loss-making format viable.

Can completed skilled care pay for the full commitments?

Completed RN visits and aide care hours earn different fees. Deduct expected credit loss once, apply payment cost to collectible earned fees, and add the actual supplies and whole-visit mileage allowances before the full paid roster and standing costs.

The default holds 30 average days of collectible earned fees in month-end receivables. Expected credit losses are excluded from that receivable rather than deducted a second time. The payment-cost estimate, supplies, mileage, paid roster and standing costs are assumed paid in the earned month; this is a conservative timing convention, not an observed processor settlement term. At a stable mature volume, the receivable stops growing, but it still ties up $50,960. A dated payroll, invoice and bank schedule can show a larger daily need.

Opening payments are approximately $72,785. The cumulative operating deficit reaches approximately $753,307 in month 60; the retained buffer is approximately $110,670. Their approximately $936,762 total would finance the modeled continuing losses over 60 months. More starting cash does not repair that operating gap. This is not a financing recommendation. Opening payments are not sustainably recovered within that horizon. Validate supported fees, service mix, territory and every qualified paid commitment before proceeding.

Test your own New Jersey skilled-care and collected-cash scenario.

Use one input set for completed RN visits, aide care hours, whole aide visit blocks, each paid role, qualified on-call availability, credit losses, collections and the full cash schedule.

The default is 80 completed one-hour RN visits at $200 per visit plus 800 completed aide care hours at $45 per hour. The fees are assumptions after concessions and refunds but before a separate 2% expected credit loss. Earning is uniform within each 30-day planning month; collections follow the entered whole/fractional lag and earlier earned-fee cohorts, with no preopening earning. Four-hour aide blocks require 200 visits; a partially used block still consumes a complete travel, documentation and handoff allowance.

RN visits are whole counts. Exact legitimate fees, care hours, rates and paid hours retain their entered precision. A dated client and staff schedule, authority for the exact services, applicable care plans, qualified clinical direction and paid 24/7 on-call arrangements can impose a lower ceiling. The director pool assumes an eligible combined clinical-direction and agency-administrator appointment only where permitted. Its 40 monthly standing hours cover governance and administration; clinical work is added once. Any combined appointment depends on the regulator’s actual employee-and-contractor count, qualifications, required direct employment, duties and qualified alternate administration. The 8.75 wage-pool FTE excludes availability-provider personnel and does not establish a statutory count. Medical administration is clerical, not the qualified administrator. Add a separately qualified paid pool where required. These inputs do not certify clinical or legal adequacy. Reset restores the exact reference.

RN visits earn a per-visit fee; aide services earn completed care hours. An aide visit block rounds up for travel, documentation, handoff and recovery even when its last care block is partly used. Credit losses reduce collectible earned fees once. Collections follow earned-fee cohorts with uniform earning in each 30-day planning month and no preopening earning. Whole and fractional collection-lag months determine the receipts; receivable movement changes cash rather than deducting fees again. The same-mix EBIT threshold exceeds at least one paid resource. More volume at this mix cannot repair the case within its capacity. The mature month loses cash. The displayed 60-month funding would finance the modeled losses; it is not a viability or financing recommendation. The 40/55/70/82/90/96/100% ramp is assumed; completed RN visits round down to whole visits and aide travel uses whole visit blocks. Qualified 24/7 on-call availability has its own retainer; field, director and alternate-response hours are distinct paid pools. Actual credentials, required plans, state authority, service territory, individual shifts, clinical need, leave, overtime, qualified backup and availability contracts require separate verification. Continuous bedside care, therapy, hospice, infusion, equipment sales and public-payer billing are excluded. Month-end cash can miss a larger daily gap. Debt, tax and owner distributions are separate.

The practical opening route in New Jersey.

Take the exact state and service area, ownership, nursing/aide categories, administrator and clinical-director credentials, client acceptance and care-plan process, operating hours, qualified on-call/alternate arrangements, field roster, insurance, records/privacy, emergency and continuity policies, office and accreditation scope to the responsible licensing authority and qualified advisers. Confirm the actual category rather than assuming every state uses the phrase home health agency for a private-pay operation.

Start with the New Jersey offices listed by the IRS
WorkstreamOfficial starting pointsWhat to ask
Business and activityWhich entity, name or activity registrations apply, and which local or specialist office also has responsibility?
TaxWhich registrations and treatment apply to the actual goods or services, location and staffing arrangements?
EmployersWhich employer accounts, reporting steps and labor obligations apply to the planned paid roster?

Take the activity, address, proposed equipment and staffing plan to the relevant office. Record applicability, supporting documents, fees, dependencies and renewal terms from the actual official response.

Agency routes were listed on the IRS New Jersey directory when retrieved September 5, 2026. Links identify starting offices; they do not verify a permit, tax treatment, fee or opening time for this business. Open the full New Jersey opening checklist →

What must be verified before opening in New Jersey?

Can the authorized RN and aide services retain completed, paid care at the realized fee mix while all field, clinical direction, availability, recovery and collection obligations fit a funded qualified roster?

  1. Define the exact licensed nursing and aide scope, accepted client needs, required plan/order and supervision, visit duration, service territory, operating hours, qualified availability, care records, cancellation/refund and payment terms. Keep the nursing visit and aide care hour separate.
  2. Track lawful referrals, inquiries, accepted clients, approved care, scheduled visits, completed RN visits, delivered aide care hours, cancellations, missed visits, actual duration and travel, concessions, expected losses, earned receivables and collections. Neither a provider fee nor an older population count establishes paid clients.
  3. Rehearse a dated client-by-client roster with qualified named staff, travel, documentation, clinical review, protected director time, leave, recovery and alternate response. Compare actual collected contribution with the complete quoted coverage cost before hiring to a larger schedule.
  1. Resolve service authority first

Confirm the jurisdiction-specific license/category, ownership and administrator/director eligibility, accepted nursing/aide scope, required plans, accreditation where applicable, insurance, records and qualified on-call arrangement before offering care. A private-pay label does not remove skilled-service obligations.

  2. Build a complete paid care route

Match authorized clients, named qualified staff, visit windows, travel, documentation, handoffs, clinical review, recovery and backup. Protect the director and alternate pools rather than adding their hours to billable field capacity.

  3. Quote and fund earning-to-collection

Obtain authority/accreditation, insurance, secure systems, staffing and availability quotes. Test separate fees and completed care, expected credit loss, dated payroll and collection lag. Revise the unprofitable reference rather than financing the same loss indefinitely.

Start with New Jersey government and agency contacts and the SBA launch guide. The relevant city, county or state office must confirm the actual activity and address. Use each cited fee or rule only for its stated activity, jurisdiction and source date. Confirm applicability and timing with the issuing office.

A fee schedule can hide a collection and capacity gap

Provider asking prices and broad surveys do not establish realized agency yield or demand. Credit losses permanently reduce collectible fees; receivables delay otherwise collectible cash. Raising volume can overbook paid caregivers before it reaches the financial threshold.

For a selected address or service area, collect an evidence pack covering premises and equipment quotes, paid demand, staffing, collection terms and the responsible authorities. The state wage and population evidence on this page does not supply those location-specific inputs.

  • BLS: wage definitions and technical notes

  • USAGov: New Jersey government and agencies

An official route to the state government and major agencies. This directory does not confirm the fees, permits or approval times for a particular address or business.

  • Reference assumptions and calculation method

All selling prices, demand, paid hours, employer allowance, premises, startup allowances, cost shares, ramp and cash buffer are authored planning inputs. No commercial quote or researched state total is implied.

State wage and population benchmarks are sourced. Confirm premises, demand, selling prices, permits and commercial quotes for the selected location. State Fit and Business Idea Scores are not assigned.
How to read the reference inputs

The stated operating format and paid roster define the scenario. Wage and population observations keep their geography and period. Replace commercial assumptions only with evidence that matches the scope, then reconcile the same inputs across earnings, cash and the delivery schedule.
