How to connect home-care authority, qualified coverage and collections
Operate from an accepted client and authorized care record linked to named qualified staff, protected clinical direction, qualified alternate/on-call coverage, completed care and dated collections. A referral, scheduled hour or advertised fee is not care delivered or cash received. The national reference pays every pool yet loses money; clinical and cash obligations must be resolved together.
What you will produce: One client-to-care-to-cash decision record covering legal service category, accepted scope and plans, qualified staff and availability, completed visits/hours, exceptions, earning, losses, receivables and the next commitment.

What to have ready
Bring actual service authority/categories, eligible leadership and staff credentials, accepted plans/orders, client terms, visit windows/territory, qualified availability contract, individual roster, completion and incident records, secure care/billing systems, insurance and dated payroll/invoice/bank records.
Work through the calculation and decision

Resolve the legal category before naming the service
Record the actual state, ownership, accepted services and payer scope before using the catalog name Home Health Care Agency. North Carolina distinguishes a Medicare/Medicaid-certified home health agency from a licensed home care agency with approved nursing and aide services. A private-pay nursing/aide business must follow the applicable licensed category without implying certification. Texas and Florida have their own categories and duties.
This reference excludes Medicare/Medicaid billing, therapy, hospice, infusion, equipment sales and continuous bedside/live-in care. Excluding a payer does not remove skilled-service authority, leadership, accreditation, supervision, records, insurance or availability duties. Obtain current applicability for the actual services, ownership and location.

Accept care through the applicable plan and qualified review
Use one client identifier linking accepted services, required orders/plan, authorized decision makers, contact, visit windows, staff qualifications, care responsibilities, supervision, agreed fees and payment/cancellation terms. Confirm which qualified professional may accept, direct, change and review care under the actual category and state rules.
Do not use a business calculator to prescribe clinical care or increase a client’s service. Visit duration/frequency, staff competence, supervision and escalation follow the accepted plan and responsible licensed professionals. The one-visit/ten-hour analytical mix is neither a treatment package nor proof that a client should receive it.

Protect direction and qualified nursing availability
Separate the field RN, clinical-director, qualified alternate-response, aide and medical-administration pools. The reference funds 40 weekly director hours, 40 field RN hours and ten alternate-response hours. A director reviewing care or an alternate handling a call cannot simultaneously be counted as the nurse delivering the same scheduled field visit. The model assumes an eligible combined qualified director/agency-administrator appointment only where permitted. Its 40 monthly standing hours cover governance and agency administration, with clinical activity added once. The medical secretary performs clerical duties, not the qualified administrator. Add a separately qualified paid pool where combination is not allowed. Any combined appointment depends on the regulator’s actual employee-and-contractor count, qualifications, required direct employment, duties and qualified alternate administration. The 8.75 wage-pool FTE excludes availability-provider personnel and does not establish a statutory count.
Texas supervising-nurse guidance requires qualified supervising/alternate availability and a written alternate designation. North Carolina guidance makes continuous RN availability during aide services consequential and its nursing checklist describes RN-on-duty coverage. The $2,500 monthly 24/7 availability retainer is a conservative authored coverage choice, separate from response work and continuous bedside care. Verify current state rules, qualified primary/alternate, restrictions, handover, absence cover, actual response pay and overtime in an executed arrangement. An allowance does not supply coverage.

Build the named client and staff route
Assign accepted care to named qualified staff, realistic visit windows and a timed route. Add direct care, travel, documentation, handoffs, clinical review, standing duties, paid breaks, relief and incident recovery. Eight hundred aide care hours become 200 four-hour visit blocks; a partly used last block retains complete noncare visit work. Monthly aggregate hours are not a simultaneous appointment schedule.
Default RN field work uses 156 of 173.3333 hours; aides use 976 of 1,040. Director, alternate and administration are independent clocks. At 85 held-mix equivalents, aide work reaches 1,035.88; the next equivalent fails at 1,047.4. Illness, skills, changing client needs, traffic, leave and actual incidents can reduce usable capacity. Revise the funded route before accepting another obligation.

Record completion, changes and exceptions once
Link each visit and aide care segment to the accepted client/plan, assigned staff, actual start/end, completed scope, paid travel/records, missed or shortened care, authorized changes and responsible review. Preserve the original event and clinical record. An incident, replacement or corrective visit does not establish a new customer sale.
The 5% RN and 2% aide incident assumptions add field recovery, clinical review and qualified alternate response. They are workload assumptions, not measured safety rates or acceptable clinical thresholds. Record actual additional work, required notifications, continuity, billing/credit treatment and authorized closure through applicable professional procedures.

Keep provider prices attached to service terms
An intermittent RN visit, extended private-duty shift and nonmedical caregiver hour have different duties and earning units. The Fort Worth/Granbury nursing range varies with duration and complexity. Raleigh’s nursing offer requires connected nonmedical-care enrollment and individual pricing. Southeast Florida caregiver offers use a four-hour daily minimum; the pricing page and FAQ show differing ranges requiring a written quote.
Retain provider, geography, worker category, duration/minimum, included work, eligibility, holiday charges, exclusions and observation date beside the offer. The authored $200 RN visit and $45 aide hour are not observed realized agency fees. A survey or provider’s advertised maximum establishes neither this agency’s client mix nor collected contribution.

Reconcile care earning to the account and bank ledger
Record each completed accepted service, fee after actual concessions/refunds, expected credit loss, collectible receivable, invoice/due date, cash receipt and remaining balance. Advances for undelivered care, unused authorized hours, cancellation charges, refunds and restricted funds stay separate. A cash receipt does not prove another visit occurred.
The reference applies a 2% loss once and then 30 average days of collectible receivables. Loss changes what can be collected; delay changes when it becomes cash. Payment cost is estimated on collectible fees and assumed paid in the earned month. Actual settlement, client disputes and dated payroll differ. Reconcile bank cash to service/account records instead of treating invoiced hours as unrestricted cash.

Quote complete authority and continuing commitments
Obtain written authority/accreditation, professional/general/worker insurance, office, installed secure records/communications, recruiting/training, field-kit and qualified availability scopes. Florida’s skilled-service accreditation duty is a consequential example: a non-skilled exemption or generic application fee cannot price the actual accreditor and accepted skilled operation.
The $65,000 opening allowance includes $10,000 for authority/policies/professional/accreditation planning without a matched quote. Deposits use cash separately from expenses; selected depreciable assets already sit in opening payments. Keep inclusions, exclusions, lead times, recurring duties and payment dates. The complete national reference loses mature cash; a larger reserve does not justify accepting the same operating loss.

Choose the next obligation from one complete record
Review accepted/completed care, clinical exceptions, qualified staff and availability, paid routes, fees/losses, receivables and bank cash together. A client or scope change should update the single calculator, staff route, care responsibility and cash schedule rather than a second partial forecast.
Before another hire, client commitment or lease, resolve applicable authority and qualified coverage, test completed care and actual collection, and show that the full funded plan can deliver the accepted services. Revise or defer when coverage or economics fails. Clinical judgment, legal applicability, demand and financial arithmetic remain separate decisions.

One client-to-care-to-cash agency record
This record structure serves the stated private-pay intermittent nursing/aide scope. It does not prescribe care or certify authority, clients or staffing.
| Record | Reference connection | Check before commitment |
|---|---|---|
| Accepted services | RN visits and supervised aide care kept separate | Actual category, plan/order and accepted scope |
| Qualified pools | 40 weekly field RN + 40 director + 10 alternate + 240 aide + 20 administration | Named eligible people, simultaneous duties, relief and overtime |
| Availability | $2,500 monthly assumed 24/7 qualified retainer; response separate | Executed primary/alternate terms and restrictions |
| Completed work | 80 RN visits + 800 aide hours; 200 aide blocks | Actual client timing, travel, documentation and recovery |
| Earning and cash | $52,000 earned fees; $1,040 loss; 30-day collectible receivable | Matched achieved fees, loss and collection dates |
| Opening scope | $65,000 allowances; authority/accreditation unquoted | Complete applicable quotes and payments |
| Decision | National case loses money even at paid capacity | Revise supported economics before expanding obligations |
What this changes: The record must explain who may provide accepted care, who delivers and supervises it, what was earned and what cash is available.
Illustrative amounts are rounded for display. Use the full inputs and stated units when recalculating; an approximate threshold is not itself a sufficient activity prescription.

Errors that can change the result
- Assuming private pay removes skilled licensing/accreditation duties.
- Calling a private-pay North Carolina home care operation a certified home health agency.
- Double-counting director or alternate hours as field capacity.
- Promising qualified availability without an executed funded arrangement.
- Treating an analytical mix as a care plan.
- Calling corrective work new demand or omitting its paid time.
- Using broad caregiver prices as RN or skilled-aide realized yield.
- Counting receivables or advances as unrestricted profit.
