Can RN visits and aide hours cover a complete home-care agency?
The authored national case earns $52,000 monthly fees from 80 RN visits at $200 and 800 aide care hours at $45. With qualified field, director, alternate, aide and administrative coverage paid, it produces approximately -$7,737 EBIT and -$7,570 cash after maintenance. Its 85-visit/850-hour paid ceiling still produces negative EBIT. More clients at the same mix cannot repair this case within capacity.
What you will produce: One calculation from completed nursing and aide care to whole visit blocks, every paid-role clock, credit loss, collections, three operating boundaries and a complete 60-month opening cash schedule.

What to have ready
Bring actual service authority, accepted nursing/aide scope and required plans/orders, qualified leadership, client/staff timetable, care duration, travel/records/recovery, availability contracts, payroll/employer quotes, separate fees, concessions/refunds, expected losses, invoices/collections and full authority/accreditation, insurance, office and system quotes.
Work through the calculation and decision
Separate nursing visits from aide care hours
One completed intermittent RN visit earns a per-visit fee; one delivered supervised aide client-care hour earns an hourly fee under the actual accepted plan, order, agreement and state authority. A referral, approved plan, booking, advance payment or unused authorization is a separate record rather than another completed sale.
Eighty RN visits at $200 earn $16,000; 800 aide care hours at $45 earn $36,000. The $52,000 total is after concessions and refunds but before a separate credit-loss estimate. One RN visit plus ten aide care hours is a held-mix planning equivalent only, not a commercial package or patient count. Actual clients may need a different approved service mix.

Convert delivered care into the complete paid field clock
Each assumed RN visit contains one direct-care hour, 0.5 travel hour and 0.25 documentation hour. Five percent require another 0.5 field-recovery hour. With 14 standing hours, RN work is 14 + 80 × 1.75 + 80 × 5% × 0.5 = 156 person-hours, within 173.3333 paid field hours.
The aide case needs ceil(800/4) = 200 whole visit blocks. Each uses 0.5 travel, 0.15 documentation and 0.1 handoff hour beyond care; 2% require another 0.5 recovery hour. With 24 standing hours, work is 24 + 800 + 200 × 0.75 + 200 × 2% × 0.5 = 976 person-hours, within 1,040 paid hours. A partial last block still incurs a complete travel/records allowance. Actual timed routes can be more restrictive.

Protect direction and qualified availability
The model separately pays 40 weekly field RN hours, 40 qualified clinical-director hours, ten qualified alternate-response hours, 240 aide hours and 20 medical-administration hours. At 52/12 weeks per planning month, RN pools total 390 person-hours. The director pool assumes an eligible combined clinical-direction/agency-administrator appointment only where permitted. Its 40 monthly standing hours cover governance/agency administration and clinical work is added once. Any combined appointment depends on the regulator’s actual employee-and-contractor count, qualifications, required direct employment, duties and qualified alternate administration. The 8.75 wage-pool FTE excludes availability-provider personnel and does not establish a statutory count. Medical administration is clerical, not the qualified administrator; add a separate qualified paid pool where required. Protected direction, alternate response and clerical administration do not add simultaneous field capacity.
The $2,500 monthly retainer is an unquoted assumption for qualified 24/7 nursing on-call availability, separate from paid response hours and excluded continuous bedside care. Default work uses 70 director, 16 alternate-response and 61.4 administrative hours. Incident-linked review and response grow with activity. Verify credentials, plans, individual shifts, restrictions, backup, leave and overtime in an executed applicable arrangement; aggregate hours do not certify adequate care.

Separate credit loss, payment cost and receivable delay
An assumed 2% credit loss reduces $52,000 earned fees by $1,040 to $50,960 collectible earning. Apply it once. The 3% payment-cost estimate uses collectible fees: $50,960 × 3% = $1,528.80. Do not deduct the same loss again from a price already adjusted for uncollectibility.
Earning is assumed uniform within each 30-day planning month, with no preopening earned fees. Collections follow the current or prior earned-fee cohorts according to the entered lag, including the fractional month. Thirty days retain $50,960 at stable mature activity. That delay is neither another expense nor another credit loss. Monthly cash subtracts the increase in collectible receivables; first-month collections can be zero under this convention. Estimated payment costs and other expenses are assumed paid in the earned month, not verified processor terms. Advances for future care, refunds, restricted funds and dated payroll need separate records.

Reconcile supplies, mileage and every fixed commitment
Supplies are an assumed $8 per RN visit plus $1 per aide care hour, totaling $1,440. Reimbursement uses 80 × 20 RN miles plus 200 × 12 aide miles = 4,000 miles at an authored $0.70 per mile, or $2,800. The rate is not an IRS, statutory or observed agency rate. Travel time in payroll and reimbursed vehicle use are different costs.
BLS May 2025 medians of $46.90 RN, $17.21 broad home-health/personal-care aide and $22.08 medical administration price $38,103 base wages. The assumed 18% employer addition gives $44,961.54 payroll. Add $2,000 office, $3,000 other standing costs and $2,500 availability retainer for $52,461.54 fixed commitments. Cross-industry medians do not quote actual qualified staffing, employer costs or availability.

Check financial boundaries against the actual ceiling
Collectible fees less payment costs, supplies and mileage leave $45,191.20 contribution. Full paid commitments produce negative $7,270.34 EBITDA. Depreciation is $28,000/60 = $466.6667, giving negative $7,737.0067 EBIT. Cash subtracts $300 capital maintenance from EBITDA rather than depreciation, giving negative $7,570.34 before receivable movement, debt, tax and distributions.
Including whole aide blocks, approximate first continuous financial conditions are 92.8822 held-mix equivalents for EBITDA, 93.4086 for maintenance cash and 93.7074 for EBIT. The first permitted whole financial cash and EBIT activity is 94 visits plus 940 aide hours. It exceeds paid capacity and is not a deliverable prescription. The calculator tests permitted activity directly because a new partly filled aide block can temporarily lower contribution.
At 85 RN visits plus 850 aide hours, 213 aide blocks use 1,035.88 of 1,040 paid aide hours. RN field work is 164.875 of 173.3333, direction 71.8775 of 173.3333, alternate response 16.51 of 43.3333 and administration 64.29 of 86.6667. The next equivalent requires 215 aide blocks and 1,047.4 aide hours, failing that pool. The ceiling still loses about $4,917 EBIT and $4,750 maintenance cash. Client compatibility, qualifications, actual routes and leave can lower it further.

Keep price context attached to its original scope
CareScout 2025 reports broad national private-duty nurse medians of $90 per hour and $160 per visit, and a $35 nonmedical caregiver hourly median. Nurse categories combine RN/LPN services and varying durations; caregiver scope extends beyond this skilled aide case. These observations are not a matched agency quote, achieved yield or paid-demand estimate.
A Fort Worth/Granbury provider describes $120–$200 nursing visits depending on duration and complexity. Raleigh nursing requires an attached nonmedical-care enrollment condition and individual quotes. Southeast Florida caregiver offers have a four-hour daily minimum and differing published ranges that need a written quote. The default $200/$45 fees remain assumptions; even a hypothetical 10% increase to $220/$49.50 leaves EBIT negative with full coverage. Do not change prices merely to force a positive model.

Read opening scope and all sixty cash months
Opening allowances total $65,000, including an unquoted $10,000 authority/policy/professional/accreditation line. The refundable office deposit uses $4,000 without becoming an expense. One hundred twenty paid training person-hours cost about $3,557, producing about $72,557 opening payments. The $28,000 depreciation base already sits within those allowances. Obtain matched complete quotes before treating any allowance as adequate.
The 40/55/70/82/90/96/100% ramp is assumed. Completed RN visits round down to whole visits; aide care scales at full precision and visit blocks round up. First-year earned fees are $536,880, collections $475,182 and operating cash -$217,509. Continuing mature losses push the deepest cumulative deficit to about $580,885 in month 60. Adding $104,923 retained buffer and opening cash gives approximately $758,366 of loss-funding over 60 months. This is not a financing recommendation; opening payments are not sustainably recovered.
The calculator, JSON and CSV use the same exact valid inputs, services, visit blocks, paid work, losses, collections and all 60 monthly rows. Blank, out-of-range inputs or fractional RN counts clear current results; reset restores the reference. Replace material care, travel, prices, qualified contracts and cash dates with applicable records before accepting another obligation. Financial arithmetic does not establish clinical adequacy or demand.

RN visits earn a per-visit fee; aide services earn completed care hours. An aide visit block rounds up for travel, documentation, handoff and recovery even when its last care block is partly used. Credit losses reduce collectible earned fees once. Collections follow earned-fee cohorts with uniform earning in each 30-day planning month and no preopening earning. Whole and fractional collection-lag months determine the receipts; receivable movement changes cash rather than deducting fees again. The same-mix EBIT threshold exceeds at least one paid resource. More volume at this mix cannot repair the case within its capacity. The mature month loses cash. The displayed 60-month funding would finance the modeled losses; it is not a viability or financing recommendation. The 40/55/70/82/90/96/100% ramp is assumed; completed RN visits round down to whole visits and aide travel uses whole visit blocks. Qualified 24/7 on-call availability has its own retainer; field, director and alternate-response hours are distinct paid pools. Actual credentials, required plans, state authority, service territory, individual shifts, clinical need, leave, overtime, qualified backup and availability contracts require separate verification. Continuous bedside care, therapy, hospice, infusion, equipment sales and public-payer billing are excluded. Month-end cash can miss a larger daily gap. Debt, tax and owner distributions are separate.

| Threshold exceeds paid capacity | Change supported mix, fees, territory or complete paid costs; do not prescribe impossible volume. |
| Availability is only an allowance | Execute and fund an applicable qualified arrangement before promising coverage. |
| Actual clinical or travel work exceeds the pool | Revise authorized scope, route or funded qualified staff before accepting care. |
| Mature cash remains negative | Do not call the 60-month loss reserve a viability or financing recommendation. |
| Collections lag earning | Fund the dated collectible receivable and payroll gap without deducting losses twice. |
Errors that can change the result
- Blending public-payer, nonmedical-only, skilled and continuous bedside formats.
- Counting director or alternate hours as simultaneous field care.
- Calling all paid caregiver time billable care.
- Using fractional visit blocks to remove travel or records work.
- Deducting credit loss twice.
- Treating provider fees or surveys as achieved yield or demand.
- Treating an availability allowance as an executed qualified contract.
- Financing ongoing losses and calling the result viable.
